COVID19

Latest update - 04/01/2022…If the client prefers, they can wear a mask in the building and during their session. Social Distancing is practiced during each behavioral health therapy session. The clients sitting area is also cleaned after each session.

Latest update - 03/04/2021...This week, Gov. Greg Abbott issued Executive Order GA-34 in which effective Wednesday, March 10, will lift all COVID-19-related restrictions on businesses and the statewide mask mandate.

However, in line with CDC guidance, all clients, visitors and staff will still be required to wear a mask in our office — we firmly stand behind these evidence-based safety measures. Our focus remains on protecting your safety and well-being.

While we continue to be encouraged by declining COVID-19 case counts and hospitalizations, coupled with increasing vaccinations, we cannot let up on the practices that have helped keep Texans safe over the last year. With a handful of new COVID-19 variants quickly spreading across our communities, it’s all the more important you continue to:

  • Mask up
  • Distance from others
  • Practice hand hygiene

Let’s continue to do our part and help stop the spread of COVID-19. 


Latest update - 6/26/2020...As per the executive order recently issued by local officials in Tarrant County, as of 6pm on Friday, June 26th, face coverings will be required in the office building, at all times, for all employees and their customers until further notice. Acceptable face covering options include homemade masks, scarves, bandanas, handkerchiefs and anything else that can appropriately cover an individual’s mouth and nose. Also note that we understand that masks can cause some problems (including anxiety) for some people with breathing issues etc. and we will be as understanding as possible in regards to the patients comfort wearing a mask, due to the circumstances, during the session.


This statement is to inform our patients about the current practices that were existing or have been implemented at the office of Stephanie Chambers, MS, LPC in Southlake, TX in regards to mental health counseling services and the COVID-19 virus (also known as the Coronavirus).

In accordance with CDC (Centers for Disease Control and Prevention) guidelines and the City of Southlake’s COVID-19 Coronavirus Disaster Declaration, we are open for in person counseling at our Southlake, Tx office as well as Telehealth video/phone counseling for anyone in the state of Texas. Please note that this can change at any time for in person counseling sessions if the CDC’s and the City of Southlake’s guidelines change accordingly for office space closures. If we do need to close the Southlake, TX office due to changes to CBC, State and Local guidelines and you are scheduled for an in person counseling session, we will inform you of the office closure as soon as possible if this happens.

Please contact us as soon as possible via phone, text or email if you would like to inquire about our Telehealth video/phone counseling services or if you would like to change a scheduled in person counseling session to a Telehealth video/phone counseling session. While most insurance plans include Telehealth services, we will still need to verify the patient’s insurance eligibility for Telehealth services before the Telehealth video/phone counseling session occurs. All private pay patients can use our Telehealth video/phone counseling services with no issues or delay right away.

In regards to preventing the spread of the COVID-19 virus, we urgently ask that you continue to use common-sense measures to keep our community safe.

Best practices and everyday preventive actions from the CDC include:

  • Wash your hands often with soap and water for at least 20 seconds. If soap and water are not available, use an alcohol-based hand sanitizer.
  • Avoid touching your eyes, nose and mouth with unwashed hands.
  • Avoid close contact with people who are sick.
  • Stay home if you are sick.
  • Cover your mouth with a tissue when coughing or sneezing, then throw the tissue in the trash. If a tissue is not available, cough or sneeze into your elbow or shoulder.
  • If you are sick with respiratory symptoms that resemble a cold or flu (e.g., fever, cough, shortness of breath), do not go to work or school or visit public places where you could spread potential illness.

In regards to the cleaning procedure put in place by the building management, they have released the following statement…

  • “The safety and wellbeing of our shopping center guests, businesses, staff and local partners are our top priority. We are closely monitoring the developments related to COVID-19 (Coronavirus Disease 2019), including updates from the Centers for Disease Control and Prevention (“CDC”), Tarrant Department of Public Health, and World Health Organization (“WHO”). We have augmented our cleaning process at the property, and continue our efforts to ensure our cleaning vendors are using cleaning agents and policies that follow the recommended CDC guidelines. We will continue to operate your property for the benefit of the community and the customers that you serve.”

In addition, we have implemented the same CDC cleaning guidelines for our office space as well. This includes:

  • Using an EPA-registered disinfectant to clean and disinfect frequently touched surfaces daily. This includes tables, doorknobs, light switches, countertops, handles, desks, and phones between each patient visit.

GFE

In accordance with the No Surprises Act, all clients that are not using insurance can request a Good Faith Estimate (GFE) for all CBHS services. Please contact us via phone, email or text to request a Good Faith Estimate. 

*Based on the No Surprises Act, starting January 1st, 2022, Chambers Behavioral Health Services (CBHS) will be required to provide a Good Faith Estimate to all clients that do not have insurance or will not be using their insurance for CBHS services. This act was created to help clients, that will not be using health insurance, understand their medical billing better and avoid surprise medical bills.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE READ IT CAREFULLY.

The Health Insurance Portability & Accountability Act of 1996 ("HIPAA") is a Federal program that requests that all medical records and other individually identifiable health information used or disclosed by us in any form, whether electronically, on paper, or orally are kept properly confidential.   • Treatment means providing, coordinating, or managing health care and related services by one or more healthcare providers. An example of this is a primary care doctor referring you to a
    specialist doctor.
  • Payment means such activities as obtaining reimbursement for services, confirming coverage, billing or collections activities, and utilization review. An example of this would include sending
    your insurance company a bill for your visit and/or verifying coverage prior to surgery.
  • Health Care Operations include business aspects of running our practice, such as conducting quality assessments and improving activities, auditing functions, cost-management analysis,
    and customer service. An example of this would be new patient survey cards.
  • The practice may also be required or permitted to disclose your PHI for law enforcement and other legitimate reasons, including emergencies and concerns regarding threats to safety. In
    all situations, we shall do our best to assure its continued confidentiality to the extent possible.

We may also create and distribute de-identified health information by removing all reference to individually identifiable information.

We may contact you, by phone or in writing, to provide appointment reminders or information about treatment alternatives or other health-related matters.

The following use and disclosures of PHI will only be made pursuant to us receiving a written authorization from you:

  • Most uses and disclosure of psychotherapy notes;
  • Uses and disclosure of your PHI for marketing purposes, including subsidized treatment and health care operations;
  • Disclosures that constitute a sale of PHI under HIPAA; and
  • Other uses and disclosures not described in this notice.

You may revoke such authorization in writing and we are required to honor and abide by that written request, except to the extent that we have already taken actions relying on your prior authorization.

You may have the following rights with respect to your PHI.

  • The right to request restrictions on certain uses and disclosures of PHI, including those related to disclosures of family members, other relatives, close personal friends, or any other person
    identified by you. We are, however, not required to honor a request restriction except in limited circumstances which we shall explain if you ask. If we do agree to the restriction, we must abide by it unless you agree in writing to remove it.
  • The right to reasonable requests to receive confidential communications of Protected Health Information by alternative means or at alternative locations.
  • The right to inspect and copy your PHI.
  • The right to amend your PHI.
  • The right to receive an accounting of disclosures of your PHI.
  • The right to obtain a paper copy of this notice from us upon request.
  • The right to be advised if your unprotected PHI is intentionally or unintentionally disclosed.

If you have paid for services "out of pocket", in full and in advance, and you request that we not disclose PHI related solely to those services to a health plan, we will accommodate your request, except where we are required by law to make a disclosure.

We are required by law to maintain the privacy of your Protected Health Information and to provide you the notice of our legal duties and our privacy practice with respect to PHI.

This notice is effective as of November 1, 2019, and it is our intention to abide by the terms of the Notice of Privacy Practices and HIPAA Regulations currently in effect. We reserve the right to change the terms of our Notice of Privacy Practice and to make the new notice provision effective for all PHI that we maintain. We will post and you may request a written copy of the revised
